Routine Uses and Disclosures: Opt-in users will receive phone calls, text messages and emails for appointment scheduling and reminders. Frequency of communication based on the need of the practice and the need of the client. Users will receive messages weekly or as needed based on their appointments. Message and data rates may apply.
